**Please Note: PRN - 8 hours per week **
Provides clinical social work and complex discharge planning to patients and their families. Holds joint accountability with case manager for facilitating psychosocial and continuing care. Works collaboratively with physicians, case managers and multidisciplinary team to effect positive patient and family outcomes. Works on an as-needed basis.
DUTIES AND RESPONSIBILITIES
1. PSYCHOSOCIAL ASSESSMENT
1.1. Performs timely psychosocial evaluations of patients and families in order to consider all variables that may impact treatment plans and outcomes, including diagnosis of emotional, social, and environmental strengths and problems related to their illness, treatment and/or life situation.
1.2. Analyzes complex data and assesses social and emotional functioning as well as patient/family adjustment to illness/disability as it impacts patient treatment, recovery and discharge plans. Demonstrates critical thinking skills in the application of clinical evidence-based services and interventions.
1.3. Demonstrates knowledge of and ability to assess age-related issues as they impact treatment, recovery, compliance and discharge plans.
1.4. Demonstrates knowledge of, sensitivity to and the ability to apply cultural, socio-economic and religious factors as they may impact patient and family.
1.5. Completes accurate, timely, pertinent, and legible medical record documentation of evaluation and assessment consis10t with department procedures.
2. PATIENT/FAMILY COUNSELING
2.1. Formulate, develop and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work diagnoses, treatments and interventions, including crisis intervention, brief individual, marital and family therapies, and patient, family and caregiver groups. Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect and domestic violence, and of mental health and/or substance abuse problems in patients and family members.
2.2. Establishes therapeutic relationships with patients and families.
2.3. Provides brief, therapeutic, goal-directed treatment and counseling services to assist patients/families to cope more effectively and to make decisions.
2.4. Appropriately documents counseling interventions and treatment services.
3. DISCHARGE PLANNING/TRANSITION MANAGEMENT
3.1. Building on the assessment process, develops, coordinates, implements and revises treatment/discharge plans for complex patients to ensure good transitional management and continuity of care. Demonstrates knowledge and understanding of all levels of post-acute care, and proficiency in facilitation of discharges to all settings.
3.2. Involves patient/family in all treatment and discharge plans.
3.3. Provides information to patient/family in a way that is clear, understandable and age-appropriate.
3.4. Completes accurate, timely, pertinent discharge planning information in the medical record.
4. MULTIDISCIPLINARY COLLABORATIVE COMMUNICATION
4.1. Develops effective partnerships with healthcare team members. Demonstrates collaboration and teamwork with case managers assigned to their units.
4.2. Participates fully and effectively in patient care, staffing and discharge planning meetings.
4.3. Actively participates in multi-disciplinary communication and team building on assigned units and in the Case Management department.
4.4. Demonstrates ability to communicate and negotiate with all levels of the department and the organization with documented information and recommended solutions.
4.5. Demonstrates constructive use of supervisory feedback.
4.6. Appropriately facilitates communication, transitions, and hand offs.
5. COMMUNITY RESOURCE LINKAGES
5.1. Demonstrates an effective community resource knowledge base and the judgment and the ability to effectively select and coordinate available resources, including referrals to regulatory agencies.
5.2. Demonstrates the ability to mobilize alternative resources to fill gaps in established community resources.
5.3. Provides consultation to case managers and healthcare team when coordination with significant or intensive community resources is necessary to achieve desired treatment outcomes.
6. PROGRAM LEADERSHIP
6.1 Demonstrates leadership and expertise in the identification of program needs, planning and goal attainment for assigned areas.
6.2 Generates and implements ideas that increase the skill level and effectiveness of social work services.

Masters Degree in Social Work from a Council on Social Work Education accredited Graduate School.
Minimum of 2 years social work experience in a health care setting.
CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED
State of Texas Social Work license.
SPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
1. Knowledge of mental health diagnoses and treatment modalities, especially as related to acute and chronic illness, crisis intervention, death and dying, etc.
2.Outstanding basic Social Work skills.
