The Pre-transplant Coordinator I facilitates and coordinates all aspects of the transplant referral, evaluation, waiting period, and peri-operative phases of transplantation. The coordinator interacts with all members of multi-disciplinary transplant team to ensure coordination of care for the pre-transplant patient.

3.Uses therapeutic communication to establish a relationship with patients and families and communicates the plan of care. Identifies and responds to the clinical, psychosocial and economic needs of the transplant patient/family/caregiver during evaluation, waiting, and pre-operative transplant phases. (EF)
4.Provides staff and community education in the areas of transplant and organ donation. Participates in department donor awareness events, educational opportunities, and community activities.
5.Presents a professional image at work and all work-related events. Fosters teamwork approach in all interactions with peers and team members. Anticipates needs of other team members; proactively offers assistance.
6.Contributes to meeting departmental and organ program targets for patient satisfaction measures on departmental dashboard.
FINANCE ¿ 10%
1.Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as UNOS/UNET, CMS, and Medicaid in order to ensure proper department financial management.
2.Contributes to meeting departmental and organ program financial measures on departmental dashboard.
QUALITY/SAFETY ¿ 35%
1.Identifies appropriate candidate upon referral and interacts with appropriate staff. Identifies requirements for candidate evaluation. (EF)
2.Identifies possible co-morbidities in the transplant candidate and evaluates as indicated. Identifies absolute and relative contraindications to transplantation. Communicates all findings with transplant team. (EF)
3.Demonstrates knowledge of, and compliance with, CMS Conditions of Participation and OPTN/UNOS policies and listing requirements. Provides data to OPTN/UNOS for listing statistics (Candidate Registration Form). (EF)
4.Recognizes potential problems, abnormal diagnostic findings, and/or significant changes during the waiting phase, coordinating required care. (Quality/Safety) (EF)
5.Exhibits ability to maintain and update candidate listing status per OPTN/UNOS regulations. (EF)
6.Provides and maintains comprehensive documentation of evaluation process, candidate progress while on waitlist, and donor/recipient transplant information. (EF)
7.Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as UNOS/UNET, CMS, and Medicaid in order to ensure compliance with all governmental reporting requirements, and proper quality outcomes monitoring. (EF)
8.Contributes to meeting departmental and organ program targets for quality and safety, on departmental dashboard.
SERVICE ¿ 30%
1.Coordinates with multidisciplinary team the evaluation process for potential transplant candidate, including, but not limited to (EF):
d.Laboratory evaluation including serologies, tissue typing, and antibodies per protocol
2.Establishes mutual educational goals with patient and family/caregivers and provides education and appropriate resources regarding (EF):
¿Living donation options (if applicable to organ)
¿Risk/benefits, including utilization of expanded donors, high-risk donors, if applicable
¿Post-operative phase including immunosuppressive therapy and effects
¿Potential research protocols of the transplant program
¿Candidate/Family/Caregiver responsibility throughout the transplant process
3.Collects and evaluates medical, psychosocial, and financial data for review by transplant team in the patient selection committee. (EF)
4.Ensures patient and referring physician are informed in writing of transplant listing, denial, or postponement. Informs and educates patient/family/caregiver at time of listing. (EF)
5.Ensures antibody screen is obtained regularly per transplant center protocol (if applicable to organ). Maintains routine health maintenance and surveillance for malignancy during the waiting period. Identifies the need for candidate re-evaluation and initiates when appropriate. (EF)
6.Participates in transplant event process according to transplant center protocol. (EF)
7.Contributes to meeting departmental and organ program targets for process on departmental dashboard.

Registered Nurse, Bachelor¿s degree required
Minimum 4 years nursing experience required. ICU experience preferred. Previous transplant acute care experience is an asset. Prior Transplant Coordinator experience not required.
CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED
Registered nurse with current Texas license
SPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
¿Skilled at managing complex patient caseload and ability to prioritize/triage based on patient status.
¿Extremely well-organized and detail oriented, self-directed.
¿Excellent interpersonal skills required to deal with diverse patient population.
¿Excellent oral and written communication skills required.
