At Houston Methodist, the Sr Pre-Transplant Coordinator position a registered nurse, responsible for facilitating and coordinating all aspects of the transplant referral, evaluation, waiting period, and peri-operative phases (as applicable) of transplantation. This position interacts with all members of interprofessional transplant team to ensure coordination of care for the pre-transplant patient. The Sr Pre Transplant Coordinator position demonstrates clinical knowledge and competence when providing care, as appropriate, using technology, and managing emergencies, acting as a patient and family advocate in order to monitor and maintain patient rights. Role modeling compassionate communication with physicians and the interprofessional health care team, this position assumes leadership responsibilities with high level contributions within the unit, service line, and across the organization. Responsibilities for the Sr Pre Transplant Coordinator position may include but are not limited to assessing department needs related to practice and improvement opportunities, serving as a clinical resource for both unit and hospital staff and coordinating unit and hospital-wide education in partnership with management.

Requirements:

PEOPLE ESSENTIAL FUNCTIONS
  • Acts as a model and coaches staff in effective verbal, non-verbal and written communication, which includes active listening and teaching. Collaborates to foster healthy relationships in the work environment such as mitigation of conflict. Advocates for a balanced workload, promotes positive behaviors, and peer-to-peer accountability.
  • Communicates candidate's status and evaluation outcome, maintaining communication while candidate on waitlist. Identifies and responds to the clinical, psychosocial, and economic needs of the transplant patient/family/caregiver during evaluation, waiting, and pre-operative transplant phases.
  • Improves nurse engagement of department by serving as a role model and mentor, facilitating teamwork with RNs, non-licensed and interprofessional staff, as evidenced by the annual RN-RN teamwork score and interprofessional relationship score.

SERVICE ESSENTIAL FUNCTIONS
  • Leads the patient and family-centered care standards, as outlined in the Houston Methodist Professional Nursing Practice document. Coordinates with inter-professional team discharge planning for the transplant recipient to provide for recipient's continuity of care
  • Develops and implements effective patient teaching strategies based on learning needs; uses appropriate resources, incorporating planning for care after discharge. Consistently evaluates the patient's comprehension and adapts teaching methods accordingly regarding Evaluation process, Organ Procurement and Transplantation Network/ United Network of Organ Sharing (OPTN/UNOS) or National Marrow Donor Programs/Foundation for Accreditation of Cellular Therapy/Center of International Blood and Marrow Transplant Research (NMDP/FACT/CIBMTR) regulations, (where applicable), organ allocation, waiting phase, transplant process, etc.
  • Collects and analyzes medical, psychosocial, and financial data for review by transplant team in the patient selection committee. Ensures patient and referring physician are informed, in writing, of transplant listing, denial, or postponement. Consistently informs and educates patient/family/caregiver at time of listing. Ensures antibody screen is obtained regularly per transplant center protocol. Maintains routine health maintenance and surveillance for malignancy during the waiting period. Identifies the need for candidate re-evaluation and initiates when appropriate.
  • Participates in transplant event process according to transplant center protocol, as applicable. Drives department activities to improve department and organ program targets for patient satisfaction, through peer-to-peer accountability.

QUALITY/SAFETY ESSENTIAL FUNCTIONS
  • Models the standards related to regulatory requirements and professional practice as outlined in the Houston Methodist Professional Nursing Practice document. Role models highly developed assessment, clinical documentation, leadership, and analytical skills; integrates and contributes to professional practice standards, policies, procedures, protocols and leads improvements in patient safety-related processes at unit or service line level.
  • Provides and maintains comprehensive documentation of evaluation process. Identifies appropriate candidate upon referral and ascertains requirements for evaluation, including possible co-morbidities, absolute and relative contraindications to transplantation, communicating all findings with transplant team.
  • Complies with and applies OPTN/UNOS/NMDP/CIBMTR/FACT policies and listing requirements, maintaining comprehensive documentation of candidate progress while on waitlist, and donor/recipient transplant information. Provides data to OPTN/UNOS for listing statistics, i.e., Candidate Registration Form, maintaining and updating candidate listing status per regulatory requirements.
  • Maintains and submits accurate data for hospital-based statistics and regulatory agencies such as OPTN/UNOS/NMDP/CIBMTR/FACT, CMS, and Medicaid in order to ensure compliance with all government reporting requirements, and proper quality outcomes monitoring. Leads initiatives to improve quality and safety scores on the unit and hospital scorecards, through peer-to-peer accountability, reporting near misses and identifying solutions by collaborating with the interprofessional team. Role models situational awareness, using teachable moments to improve safety.

FINANCE ESSENTIAL FUNCTIONS
  • Models independent time management effectively and prioritization of daily tasks. Actively participates in the stewardship and efficient utilization of resources and needs including unit capital and operational budgets. Validates patient's insurance authorization for applicable diagnostic testing, ensuring documentation of applicable clinical activities for department charge capture and proper department financial management.
  • Implements department strategies to achieve financial target and organ program financial measures and mentors others to do the same through timely documentation of care, optimizing efficiency, and other areas according to department specifications.

GROWTH/INNOVATION ESSENTIAL FUNCTIONS
  • Leads and identifies opportunities for practice changes and performance improvement. Investigates and incorporates transplant-specific evidence-based practices which are presented to department and leadership. Supports change initiatives and adapts to unexpected changes.
  • Expands individual nursing knowledge and coaches staff to grow in knowledge, abilities, skills, and attitudes. Serves as the clinical resource. Incorporates evidence-based practice to daily work, such as reviewing the professional literature, attending educational opportunities both formal and informal, i.e., CMPI and professional meetings. Develops and disseminates, as appropriate, informational/educational resources and programs designed to improve quality and professional practice. Completes and updates the My Development Plan on an on-going basis.

Qualifications:

EDUCATION
  • Bachelor's degree or higher from an accredited School of Nursing

WORK EXPERIENCE
  • Two years Transplant Coordinator experience
LICENSES AND CERTIFICATIONS - REQUIRED
  • RN - Registered Nurse - Texas State Licensure and/or Compact State Licensure within 60 days OR
  • RN-Temp - Registered Nurse - Temporary State Licensure within 60 days AND
  • BLS - Basic Life Support (AHA) AND
  • Magnet - ANCC Recognized Certification -- Certified Clinical Transplant Coordinator (CCTC), OCN, or BMTCN for the appropriate clinical service line OR Ventricular Assist Device Certification Board (VAD-CB)-recognized certification - Ventricular Assist Device Certification (VAD-C¿) for the appropriate clinical service line

  • Ability to provide assistance to physicians in perioperative setting
