JOB SUMMARY:Ensure optimal medical nutrition therapy through individualized screening and nutritional assessment, development and implementation of medical nutrition therapy care plans, nutrition education, and monitoring through reassessment of response to nutrition care. Responsible for development of transitional feeding plan if indicated and termination of nutrition intervention when clinically appropriate. Ensure positive impact on quality and cost outcomes by assuring optimal medical nutrition therapy to minimize potential complications related to alterations of nutrition status.

Responsible for screening, assessment, reassessment and medical nutrition therapy. Oversee nutrition screening and assess nutritional status of patients identified at nutritional risk by evaluating: age, gender, diagnoses, past medical & surgical history, drug-nutrient interactions, food allergies, cultural/religious food preferences, etc.; individualized nutritional needs; anthropometric measurements; biochemical/microbiological indices; organ system function; physical assessment; diet history; medications; alterations of nutrient digestion, absorption or metabolism; socioeconomic status. Develop and implement medical nutrition therapy care plan. Ensure safe, cost-effective & appropriate medical nutrition therapy. Recommend and implement appropriate nutrition care. Educate patient, caregiver, interdisciplinary team and intradepartmental team on nutrition information. Assess effectiveness of nutrition education and evaluate response to nutrition care for patients identified at nutritional risk. Verifies written diet/nutrition support order prescribed by physician and evaluates clinical effectiveness and progress of nutrition care. Facilitate communication of medical nutrition therapy care plan to assure continuity of care between patient care sites. Suggest evaluation by adjunctive services as indicated and conduct training and mentorship when necessary. Maintain annual professional portfolio for professional growth and complete self-evaluation for annual performance appraisal. Complete quality monitors and activity/ productivity & other reports on timely basis. Identify and document evaluation of outcomes and demonstrate the components of the ICARE values statement. Demonstrate SERVICE PRIDE standards.

MINIMUM REQUIREMENTS:

Bachelor’s degree in dietetics, food science, nutrition, institutional management or relevant field from an accredited university, and completion of dietetic internship accredited by the Academy of Nutrition and Dietetics.

CERTIFICATES, LICENSES & REGISTRATIONS:

 

Registered Dietitian certified by the Commission on Dietetic Registration and Dietitian License by Texas Department of Licensing and Regulation.
